[1]-Choose the Sphere and create spehere with the following size
Place the box like me 
[2]-Create box and cylinder with the following size and place like me as shown below 
[3]-Create line and open the rendering palette now use the following setting
Now create shape like me 
[4]-Convert the line into the editable spline and select the vertex now apply the fillet 6.0 
[5]-Now convert the line into the editable poly select the vertexes now drag like me 
[6]-Open the rendering palette now use the following setting and create circle with the following size
Place the circle like me 
[7]-Make the materials like me 
[8]-Now you are done it enjoy the final output as shown below
