[1]-First of all Open New Document of Custom Size, Height & Width 300 x 300 pixels,Resolution 72 & Mode RGB Color
[2] Take the Rounded Rectangle Tool apply the following settings.
[3] Create a shape the color is #c9d2db.
[4] Go to Edit > Transform > Perspective, Now move the points so they look like this.
[5] Add the following layer styles.
[6] Create a shape with the help of Elliptical Marquee Tool fill that layer with black.
[7] For this shape add the following layer styles.
[8] Take the Ellipse Tool create a shape then press Cntl+T, right click to select Warp apply these settings.
[9] Right click,select Make selection
[10] Now create a new layer make sure your forground is set to white Now select the gradient tool and set it to Foreground to Transparant and drag it from top to bottom.
[11] Create a shape with the help of Elliptical Marquee Tool Now create a new layer make sure your forground is set to white Now select the gradient tool and set it to Foreground to Transparant and drag it from top to bottom.
[12] Create another small shape with the help of Elliptical Marquee Tool.
[13] Create a shape with the help of Elliptical Marquee Tool, now fill that layer with #747b83.
[14] Create another shape with the help of Elliptical Marquee Tool, now fill that layer with #ccd4d7.
[15] Now create the following selection with the help of Pen Tool, fill that layer with Black.
[16] Create a more shape like this.
[17] Create a shape with the help of Elliptical Marquee Tool, create a new layer above the 1 layer and fill that layer with #525355.
[18] Now set the Opacity 21%.
[19] This is my final result.
