[1]-First of all Open New Document of Custom size,Height & Widht 1024×760 pixels,Resolution 72 & Mode RGB Color

[2]-Now Open New Layer & then fill with any color & then go to Blending Options,select Gradient Overlay & use the following
settings

[4]-Now fill the selction with ‘cccccc’color & then go to Filter>Artistic>Sponge & use the following settings

[5]-Now your image should look like as shown below

[8]-Now go to Filter>Distort>Spherize & use the following settings

[9]-Now press Ctrl+F for best appearance & then Reduce the following shape with the help of Ctrl+T & then press ENTER

shape with the help of PEN TOOL,right click select Stroke Path
