[1] Take a new file of 300 pixels, 350 pixels, of resolution 72 dpi in the RGB mode.
[2] Take the Pen Tool create a shape like this.

[4] Double click the layer to open layer style window. Apply settings as shown.

[6] Now make the copy of the shape layer, and clear layer sytle, then press cntl+t, free transform to minimize your shape like me.

[9] Select the Elliptical Marquee Tool create a shape, and fill that layer with Black.
[10] Double click the layer to open layer style window. Apply settings as shown.

[12] Now select the Rectangular Marquee Tool create a shape and fill that layer with Black.
[13] And now change the Blending Mode to Soft Light.
[14] Select the Elliptical Marquee Tool create a shape, and fill that layer with Black.
[15] Double click the layer to open layer style window. Apply settings as shown.
