[5] Apply the following layer styles.

[7] Take the Rectangle Tool create a shape, the color is #ecd36e, Hit CTRL-T and choose Perspective Adjust the nodes to get the shape as shown.
[8] For this shape add the following layer styles.
[9] Duplicate your shape layer, Hit CTRL-T and choose Flip Horizontal, then minimize your shape shape like this.
[10] This is my result.
[11] Create two shape with Ellipse Tool.
[12] For these both shape add the following layer styles.
[13] This is my result.
[14] Take the Brush Tool use these settings, after that set the foreground color.
[15] Draw shape line like me.
[16] Now add the following layer styles.
[17] This is my result.
[18] Select the Rounded Rectangle Tool create a shape.
[19] For this shape add the following layer styles.
[20] Take the Text Tool write your text.
[21] Select both layer shape layer and text layer Hit CTRL-T and choose Rotate 90CW.
[22] This is my final result.
